shall be completed in writing, under oath and subject to penalties of perjury, for each person signing the application and each manager employed or to be employed at the licensed premises. A complete application for any license renewal (i.e., liquor license application and all liquor license personal interest forms, and all attachments thereto) shall be submitted at least ninety (90) days prior to the beginning of any license year.
